Please call our customer service department to have current order form mailed or faxed to you.
CANCELLATIONS: All cancellations must be made in writing. Any costs incurred will be billed to your account.
An order will only be considered complete if the entire order form has been completed including the following information:
* Your name and company name * Purchase order number * In hand date *Method of shipment (overnight, 2-day air, ground service 3-5 days) * Complete address or complete list of drop shipment addresses * Approved credit, prepaid check, deposit or credit card number
IMPRINTING PRICES INCLUDE: Gold foil hot stamp on white paper and personalized label. 601A: Other foil colors are available for a $25.00 setup.
HOT STAMP FOILS AVAILABLE: Gold, silver, cherry red, teal, black, burgundy, mint green, medium blue, fuchsia, rainbow, baby blue, purple, pink, emerald green, violet, periwinkle, royal blue, and hunter green.
